			<content:encoded><![CDATA[<p>Amgen Inc, aiming to secure future sales of its flagship anemia drug Epogen as potential competition emerges, has reached new supply contracts with the two largest operators of U.S. kidney dialysis clinics. The biotechnology company said in a regulatory filing on Friday it signed a seven-year contract to provide DaVita Inc with at least 90 percent of the clinic operator&#8217;s anemia drug needs.</p>
<p>Amgen also entered into a &#8220;multi-year&#8221; agreement, starting in January, to supply Epogen on a &#8220;nonexclusive&#8221; basis to Fresenius Medical Care, according to Amgen spokeswomen Christine Regan. Fresenius has a U.S. dialysis market share of around 33 percent, while DaVita&#8217;s share is about 26 percent. Both contracts include discounts and rebates.<br />
<span id="more-27"></span><br />
&#8220;Amgen has effectively locked in two-thirds of its Epogen customer base with these DaVita and Fresenius contracts,&#8221; Stifel Nicolaus analyst Maged Shenouda said in a research note, adding that the deals are likely structured to give bigger discounts based on volume and competitor pricing.</p>
<p>Epogen sales, which totaled $2.52 billion in 2010, have waned in recent years due to safety concerns. But Amgen still holds a virtual monopoly on the market for drugs used to boost red blood cell levels in kidney dialysis patients. That could change as generic, or &#8220;biosimilar,&#8221; versions of such erythropoiesis-stimulating agents reach the market and competitors such as Roche Holding AG start selling similar drugs.</p>
<p>Another potential rival is Affymax Inc, which is seeking U.S. regulatory approval for its experimental drug, peginesatide, as a treatment of anemia in kidney dialysis patients. A panel of expert advisers to the U.S. Food and Drug Administration is set to review the drug, developed by Affymax and Takeda Pharmaceutical Co Ltd, on December 7, with a final decision from the agency expected by March 27.</p>
<p>Should it win approval, Affymax is expected to price its product below Epogen.</p>
<p>The Medicare federal health insurance program began this year to reimburse dialysis clinics based on a fixed amount for each patient, rather than the previous method of simply marking up from costs &#8212; which critics argued led to overuse of drugs, including Epogen. Medicare, which traditionally covers elderly and disabled Americans, provides coverage for all those with end stage renal disease under a decades-old law.</p>
<p>&#8220;Amgen might have agreed to match or partially match competitors&#8217; epo pricing on the logic that taking (DaVita) out of the pricing picture would help preserve a far higher price,&#8221; ISI Group analyst Mark Schoenebaum said in research note. RBC Capital Markets analyst Michael Yee expects Amgen&#8217;s sales of Epogen to fall 16 percent in 2012 and 11 percent in 2013.</p>
<p>The supply deals do not involve Amgen&#8217;s second-generation anemia drug, Aranesp, which generated $2.49 billion in sales last year. Shares of Amgen, based in Thousand Oaks, California, fell 0.6 percent to close at $55.57 on Friday in Nasdaq trading.</p>

<p>In documents released on Tuesday, staffers from the Food and Drug Administration said they were especially worried about the high rate of stroke for patients who used Medtronic&#8217;s Cardiac Ablation System to treat their persistent atrial fibrillation (AF), a major cause of stroke.<br />

Medtronic shares fell 0.9 percent to $34.40 in morning trading on the New York Stock Exchange, in line with a 1 percent drop in the S&#038;P Health Care Equipment index.</p>
<p>Atrial fibrillation, which affects more than 2 million Americans, is the most prevalent heart rhythm disorder, in which the heart&#8217;s upper two chambers contract rapidly and irregularly, allowing blood to pool. This can form clots that travel to the brain and cause strokes.</p>
<p>The Medtronic device treats persistent AF, which consistently lasts for more than seven days, or recurs for as long as four years. The company said there are currently no FDA-approved devices to treat this condition.</p>

The new laws, which will force cigarettes to be sold in plain packaging from 2012, are being closely watched by New Zealand, Canada, the European Union and Britain, which are considering similar restrictions.</p>
<p>Health Minister Nicola Roxon said the conservatives, who managed to postpone an upper house Senate vote on the bills during a rancorous day in parliament for the minority Labor government, were playing into the hands of big tobacco firms. &#8220;Given the delays in passing the bill caused by the opposition, the government now has no choice but to reconsider the impact on implementation timeframes,&#8221; Roxon said.<br />

Australia says the new laws reflect its obligations under the World Health Organization&#8217;s 2005 framework against tobacco, which urges states to consider plain packaging laws. The WHO estimates more than 1 billion around the world are regular smokers, with 80 percent in low and middle income countries. The laws have angered tobacco producers who have threatened a High Court challenge, while the governments of Nicaragua and Ukraine said the new measures breached international trade rules and would be challenged in the World Trade Organization.</p>
<p>Prime Minister Julia Gillard&#8217;s government is hoping the laws will come into effect on January 1 next year, although the main provisions forcing all cigarettes to be sold in plain packaging only come into operation on July 1, 2012. The delay in the Senate means the laws will not be voted on before November, forcing the government to reconsider now whether it can meet its deadlines.</p>
<p>Australia&#8217;s Cancer Council said the Senate should end the political delays and get on with passing the legislation, with authorities estimating smoking now kills 15,000 Australians each year and costs the health system $32 billion. &#8220;We know the tobacco industry is vehemently opposed to plain packaging, which is just another indication that plain packaging has great potential to reduce tobacco,&#8221; council chief executive Ian Olver said.</p>
<p>Analysts say tobacco companies like Britain&#8217;s Imperial Tobacco and Philip Morris are worried that plain packaging could spread to emerging markets like Brazil, Russia and Indonesia, and threaten growth there. Australia&#8217;s tobacco market generated total revenues of around $10 billion in 2009, up from A$8.3 billion in 2008, although smoking generally has been in decline. Around 22 billion cigarettes are sold in the country each year.</p>
<p>British American Tobacco, whose brands include Winfield, Dunhill and Benson &#038; Hedges, has said the government&#8217;s plans would infringe international trademark and intellectual property laws, promising a court challenge to the laws.</p>

			<content:encoded><![CDATA[<p>A new study suggests Americans with heart failure may benefit less from recommended medications than patients in other countries. The findings, which one doctor called &#8220;provocative,&#8221; point to an emerging problem in drug development: As research becomes increasingly international, the outcomes may be less dependable in individual countries. In the new work, researchers drive that point home for drugs known as beta blockers, which gold-standard tests have shown can extend the life of people with heart failure.</p>
<p>Breaking those drug trials down by country, it turns out U.S. patients didn&#8217;t benefit appreciably, whereas the number of deaths during the trials fell by more than five in 100 in the rest of the world. Lead researcher Dr. Christopher O&#8217;Connor, a cardiologist at Duke University Medical Center in Durham, North Carolina, said earlier studies had hinted of geographical differences in outcomes, but never as clearly as the new results.<br />
<span id="more-12"></span><br />
Some five million Americans are living with heart failure, in which the heart fails to pump enough blood into the body. The condition is involved in about 300,000 deaths every year, according to the National Institutes of Health. O&#8217;Connor&#8217;s study, released Monday in the Journal of the American College of Cardiology, represents the first sweeping look at how beta blocker trials may come to different conclusions depending on the nationality of the participants.</p>
<p>He and his colleagues pooled results from so-called randomized controlled trials &#8212; the strongest experimental design available to doctors &#8212; that included U.S. patients. They found four trials &#8212; the MERIT-HF, the COPERNICUS, the CIBIS-2, and the BEST trials &#8212; with a total of some 9,000 participants, nearly half of them American. During those studies, beta blockers cut deaths among non-U.S. patients by 36 percent, while there was no statistically reliable drop among U.S. patients.</p>
<p>While that doesn&#8217;t necessarily mean Americans don&#8217;t benefit at all, it does question heart failure treatment guidelines, which often rely on international trials.</p>
<p>&#8220;It raises some concerns,&#8221; O&#8217;Connor told Reuters Health. &#8220;If the patients have these differential treatment effects, we have to pay attention to it.&#8221; There aren&#8217;t any clear-cut answers to why the outcomes vary by country. It could be a question of different use of medications or devices to control heart failure, or it could be a chance finding because only one study included a large proportion of U.S. patients, the researchers write.</p>
<p>O&#8217;Connor said genetic differences might also be at play, because African Americans more often harbor a gene variant that makes them less responsive to beta blockers than whites. His study was funded in part by ARCA biopharma, which is developing a genetically targeted beta blocker treatment.</p>
<p>Whatever the explanation for the geographical variation, it&#8217;s a problem drug developers would do well to tackle if they want their products to be approved by the U.S. Food and Drug Administration, according to O&#8217;Connor. &#8220;This really goes beyond the beta blocker and heart failure story, it&#8217;s about how we conduct clinical trials,&#8221; he said. &#8220;I think the FDA gets nervous when they keep seeing these trends where, things may not look as good in the U.S.&#8221;</p>
<p>As a case in point, for a while it appeared the FDA might not approve drugmaker AstraZeneca&#8217;s new blood thinner Brilinta, because North American patients taking the drug did worse than patients on an older blood thinner &#8212; which wasn&#8217;t the case for European patients in the study.</p>
<p>AstraZeneca finally convinced the FDA the reason for the poorer results in North America might be related to the fact that aspirin interferes with Brilinta, and aspirin is more commonly used by heart patients in the U.S. than in Europe. The FDA approved Brilinta on July 21. In an editorial in the same journal, Dr. Barry M. Massie, explains that many drug trials have now become &#8220;megatrials,&#8221; including several thousand patients in different countries.</p>
<p>That&#8217;s partly because current drugs and medical devices are so effective now that most advantages from new treatments will be miniscule. As a result, it requires lots of patients to show that potential benefits aren&#8217;t just a result of chance. While Massie, of the VA Medical Center in San Francisco, calls the new findings &#8220;provocative,&#8221; he adds that American patients might still benefit from beta blockers.</p>
<p>&#8220;Most important, one cannot exclude the play of chance in these findings,&#8221; he writes.</p>

			<content:encoded><![CDATA[<p>Buoyed by trials of drugs to prevent the spread of HIV, a global AIDS conference on Monday debated how to draft recommendations and muster funds to transform these dramatic results into action in the field. Researchers from around the world were getting their first look at full peer-reviewed data from a trial that, say veteran campaigners, could slow the 30-year-old juggernaut of AIDS.</p>
<p>It was carried out among 1,763 couples where one partner was infected by the human immunodeficiency virus while the other was HIV-free. When the infected partner was given an early start on HIV drugs, this slashed the risk of transmitting the virus to the other by 96 percent &#8212; a figure readily comparable to the effectiveness of a condom.<br />

Other trials, in which the non-infected partner took the drugs, found a risk reduction of up to 73 percent.</p>
<p>Even battle-hardened veterans in the war on AIDS are stunned.</p>
<p>Antiretroviral drugs which are a lifeline to millions of infected people are poised for a glittering new role, as a tool to prevent viral spread, they say. &#8220;We are at an important tipping point,&#8221; Anthony Fauci, director of the US National Institute of Allergy and Infectious Diseases (NIAID), told AFP.</p>
<p>&#8220;There is an extraordinary window of opportunity and the sooner we act on it and the more robustly we act on it, the quicker we&#8217;ll achieve the goal of turning around the epidemic. &#8220;Pure, simple math tells you that the fewer people who are infected, the fewer the people who are going to get infected. We&#8217;ve got to turn that knob and get it on the right trajectory.&#8221;</p>
<p>But Fauci and others said many tasks lay ahead in safely transposing a trial, which takes place in carefully controlled conditions where couples are counselled and supported, into the messy reality of everyday life. Reflecting the historic turn of events, the UN&#8217;s World Health Organisation (WHO) on Monday scrapped plans to issue new guidelines on administering antiretrovirals, said delegates.</p>
<p>These will now be reviewed in the light of the latest discoveries, they said. &#8220;People will be arguing about the next six or eight weeks about what to do, the maximum transmission benefit and the maximum treatment benefit, where do they overlap,&#8221; said Myron Cohen, lead investigator of the landmark HPTN 052 trial and professor of microbiology and immunology at the University of North Carolina at Chapel Hill.</p>
<p>&#8220;There will be formal recommendations and there will be momentum, the two things will go in parallel.&#8221; Another question, though, is money. After a surge in funding in the last decade, support for AIDS from rich countries has plateaued. Poorer countries are doing more, but there is still a huge gap.</p>
<p>Overall, around 16 billion dollars have been earmarked this year, compared to needs estimated at 22 to 24 billion dollars depending on the treatment.</p>
<p>Michel Kazatchkine, head of the Global Fund to Fight AIDS, Tuberculosis and Malaria, turned a worried eye to this problem. &#8220;The priority right now is reaching people who really need treatment,&#8221; he said in an interview, referring a threshold defined by current WHO guidelines as below 350 CD4 immune cells per microlitre of blood.</p>
<p>&#8220;There will be spectacular progress over the next five years. We will practically be reaching universal access&#8221; to people in this category, Kazatchkine told AFP.</p>
<p>&#8220;But putting everyone on treatment is not a prospect, either from the resource point of view or from the operational point of view. Half of the people who are infected aren&#8217;t even aware of their (infection) status.&#8221; AIDS has killed around 30 million people since it was recorded as a novel disease in 1981.</p>
<p>More than 34 million people were living with HIV, two-thirds of them in Africa, at the end of 2010, according to UNAIDS. The conference gathers 5,500 specialists, ranging from virologists to pharmacologists and disease trackers. It is staged once every two years by the International AIDS Society (IAS), which also organises the International AIDS Conference, a bigger event that touches on the pandemic&#8217;s many social dimensions.</p>
